<h3 style="color: #b40827;">Why straighten teeth?</h3>
<p>The most common reason for wanting to straighten teeth is of course to look good.</p>
<p>There is no denying that a straight row of teeth will give a more attractive smile.</p>
<p>Apart from looking good, straight teeth are also easier to keep clean. This, in turn makes it less likely for your teeth to have decay or gum disease. Straight teeth also lead to better bites – that is, the upper and lower teeth bite more efficiently.</p>
<h3 style="color: #b40827;">The methods available to straighten teeth</h3>
<p>The traditional way of straightening teeth have been with <a href="../service/braces/">braces</a> – in either metal or ceramic. Braces are actually little brackets that are fixed to the teeth. Then, special wires tied into the brackets gently move the teeth into place. The wires need to be adjusted every 4 weeks or so. Certainly, some time is needed to get used to the braces and metal in the mouth.</p>
<p>There are many adults who are unhappy with the arrangement of their teeth. What holds them back from improving their teeth is the thought of wearing braces and metal wires. Now, with the technology of aligners, it is possible to improve the alignment of teeth <u>without</u> braces or wires. There are several companies providing such aligners, the most established brand being <a href="../service/invisalign/">Invisalign</a>.</p>
<p><a href="../service/invisalign/">Invisalign</a>, uses a series of custom-made, transparent, removable aligners that fit over the teeth – like contact lenses for the teeth! Each aligner is worn for about 2 weeks and is designed to move the teeth a little at a time. The system uses its patented CAD-CAM technology and a clinical software that allows the dentist to design the movements of the teeth.</p>
<p><strong><span style="color: #b40827;">The main advantages</span></strong> of aligners are that they are:</p>
<ul>
<li><strong><span style="color: #b40827;">Clear</span></strong> – people don’t need to know you are straightening your teeth</li>
<li><strong><span style="color: #b40827;">Convenient</span></strong> – the aligners are removable, allowing you to eat whatever you want and making it is easy to clean your teeth.</li>
<li>More <strong><span style="color: #b40827;">Comfortable</span></strong> – no sharp braces or wires to get used to</li>
</ul>
<p><strong><span style="color: #b40827;">What will treatment with Invisalign aligners be like?</span></strong> The aligners need to be worn about 20-22 hours a day. i.e. they are only removed for eating and cleaning. Each aligner needs to be worn for a minimum of 2 weeks before changing to the next aligner in the sequence. Patients need to be reviewed by the dentist every 4-6 weeks to ensure that the teeth are moving according to plan. As there are no wires to change or adjust, each appointment is usually much faster – and less painful – than with braces. Just like with braces, treatment time varies with the complexity of the case.</p>
<p><strong><span style="color: #b40827;">Is it for Everyone?</span></strong> The <a href="../service/invisalign/">Invisalign</a> system of aligners is suitable for adults &amp; older teenagers who have mild to moderate irregularities of their teeth. It is particularly suitable for those who have had treatment with braces in their teens but now have mild relapse.  <a href="../service/braces/">Braces</a>, on the other hand are much more versatile and can treat a wide variety of problems. In cases of severe crowding, where teeth need to be extracted, it is possible that treatment with Invisalign will take longer.</p>

<p>There are other issues to consider, such as your lifestyle or habits &amp; your expectations.</p>
<p>Fixed braces cannot be removed, so cleaning may be more difficult. This is a particular concern for those adults who are susceptible to gum disease and need to practise a high level of hygiene. Invisalign is removable so cleaning teeth is much easier. On the flip side, a high degree of discipline is needed to make sure the appliance is worn diligently according to instructions. Invisalign is best for people for whom it is necessary that their treatment is discrete.  Your expectations and the demands of your work or social life will influence your decision on whether you choose braces or aligners.</p>
<p><strong><span style="color: #b40827;">What is the first step?</span></strong> If you want to know whether braces or aligners treatment is appropriate for your condition, you need to have an orthodontic consultation. At this consultation, your records like X-rays, photographs and moulds of your teeth will be made by the clinic. One advantage of Invisalign is that a computerised simulation of how your teeth will move during treatment, and the expected end-result, can be viewed by you to help you make a decision. Only after you agree to treatment and give the go-ahead, will the Invisalign aligners be manufactured.</p>

<h2 class="bold m-t-10">Conditions when your wisdom tooth can be left alone:</h2>
<ul>
<li>The wisdom tooth is not visible in the mouth at all, i.e. it is completely buried in the jawbone. Then chances are the tooth can be left alone. It is wise to monitor the tooth by taking X-rays periodically to make sure that problems like cysts do not occur around the buried tooth.</li>
<li>The wisdom tooth has grown completely into the mouth and is in good alignment with the other teeth and can be easily cleaned with your toothbrush and floss</li>
</ul>
<h2 class="bold m-t-10">Conditions when your wisdom tooth needs to be removed:</h2>
<ul>
<li>The wisdom tooth has partially grown into the mouth and then gets stuck (impacted) because of lack of space in the jaws.</li>
<li>The gums around the wisdom tooth gets infected (pericoronitis)</li>
<li>The wisdom tooth has decay</li>
<li>The wisdom tooth affects adjacent teeth e.g. accumulation of food debris between the impacted wisdom tooth and the tooth in front of it leads to tooth decay. The decay can often go undetected as in the early stages there may be no pain. (Please see diagrams below)</li>

<p>This is usually the <em><strong>first question</strong></em> that patients ask when they want to <a href="../services/braces/"><em><strong>straighten teeth</strong></em></a>.</p>
<p>Fees vary greatly according to the condition to be treated, time taken, the expertise of the dentist and methods used for treatment. For example, treatment fees can range from as little as $3500 for simple cases to $20,000 or more if surgery is needed.</p>
<p>Although cost is an important factor in decision making, there are many things your dentist needs to know before a <em><strong>treatment plan </strong></em>and a <em><strong>fee</strong></em> can be given to you.</p>
<p>For example,</p>

<h3><strong>What is the nature of your problem?</strong></h3>
<p>There may be some conditions that cannot be fully corrected with braces alone e.g. jaw proportions. Before you start any treatment, it is necessary to understand the nature of your condition, the limitations and risks i.e. what treatment can and cannot achieve.</li>
<li class="listLiStyle">
<h3><strong>What do I need to do to maximise the effectiveness of treatment?<br />
</strong></h3>
<p>Treatment done at the right time e.g. in growing children, can greatly maximise the effectiveness of treatment. In other conditions, treatment may need to be done in phases, e.g. gum treatment first before proceeding to straighten teeth.</li>
<li class="listLiStyle">
<h3><strong>What are the alternatives?<br />
</strong></h3>
<p>Ask if there are alternative ways to getting the benefits that you want. When you are convinced about the choice of treatment, you will be both motivated and enthusiastic about the treatment.</li>
<li class="listLiStyle">
<h3><strong>What is the appropriate treatment for me?</strong></h3>
<p>In order to give the treatment appropriate for you, the dentist will have to examine you, assess the nature or severity of the problem, gather information through X-rays, photos and models of your teeth. Only then, can a treatment plan be offered.</li>
</ul>
<p>Always ask for written documentation of payment terms. Some clinics may be willing to collect fees from you on a month to month basis but more clinics are moving away from this as collection of payments can be bothersome and lead to conflicts if patients do not attend regularly for treatment. Smart patients charge the full fee to credits cards that allow them to pay monthly at little or even no interest.  Ask your dental clinic which credit cards they extend this benefit for – and apply for that card!</p>
<p><strong>Bottom line</strong>, just like a renovation contractor would need to know the condition of your house, your preferred design, the materials you like and your time frame before quoting you a fee, a dentist too needs to understand the nature of your problem, your desires etc. Only then, can a quotation for the fee be given.</p>

<h2 class="bold">Bruxism</h2>
<p>This refers to forceful grinding or clenching of the teeth, often unconsciously, during the day or at night during sleep. Many people who brux their teeth may not be aware that they are doing so. However, those who sleep with them may be able to hear their grinding. The exact cause of <a href="http://www.nlm.nih.gov/medlineplus/ency/article/001413.htm" target="_blank" rel="noopener">bruxism</a> is uncertain but mental stress is considered one of the factors contributing to this habit.</p>
<p>Unconscious clenching or grinding can lead to problems like:</p>
<ul>
<li>Cracked teeth</li>
<li>Teeth sensitivity due to wear of the teeth</li>
<li>Headaches, or tiredness of the jaws on waking</li>
<li>Flattened or chipped appearance of the teeth</li>
</ul>
<p>As there may be many causes for bruxism , a holistic approach is needed to treat this condition. Some people may find that wearing an occlusal splint or <a href="http://www.nhs.uk/Conditions/teeth-grinding/Pages/Treatment.aspx" target="_blank" rel="noopener">night guard</a> may help to reduce the clenching and consequent wear on the teeth.</p>

More often, it may be caused by medications taken to treat stress, anxiety or depression. Reduction of <a href="https://www.smilemakers.com.sg/saliva/">saliva</a> in the mouth can have many consequences such as:</p>

<li>Reduced taste and hence reduced appetite</li>
<li>Increased tooth decay because of the lack of protection offered by saliva</li>

People under stress are less likely to eat well, sleep well or be interested in daily hygiene practices; hence their general resistance to infections fall. <p>Yellow or discolored teeth can be treated with whitening agents containing carbamide peroxide. Applied directly on the surfaces of the teeth, carbamide peroxide treatments can be done at home. The dentist will construct a customized tray for the patient to hold the whitening agent and advise the patient as to how long the appliance should be worn each day and the duration of the treatment. Customized dental trays help prevent the gel solution from coming in contact with soft tissue, which can cause sensitivity and irritation.</p>
<p>While there are many over-the-counter whitening agents, these are of much lower concentration than those prescribed by a dentist, and not all discolorations respond to treatment. Hence it is necessary to have a consultation with a dentist to see if you are suitable for the whitening procedure.</p>

<h3>How long will the teeth stay white?</h3>
<p>This depends on your habits like how effectively you brush your teeth, how much coffee, tea or wine you drink and whether you smoke.</p>
<h2>Will teeth-whitening work for everybody?</h2>
<p>Unfortunately no.</p>
<ul>
<li>Teeth Whitening products work best on natural teeth that are completely sound i.e. with no fillings or crowns.</li>
<li>People with deep stains caused by antibiotics may see very little improvement.</li>
<li>Teeth with uneven colours e.g. opaque white spots, cannot be changed to uniform colour.</li>
<li>People with gum recession and exposed roots, or those who have worn out teeth or faulty fillings, may feel sensitivity</li>
</ul>
<p>For these reasons, it is best to have a professional consultation to find out which method is best for you. Sometimes all you may need is a good, professional polishing to have brighter teeth!</p>

Depending on the severity of the disease, treatment may involve sessions of deep scaling and root planing to remove calculus (hardened plaque) accumulated below the gums. Careful removal of unhealthy tissues followed by diligent hygiene at home can greatly improve the condition of the gums. Antibiotics and antiseptic mouthwashes may be prescribed to reduce the bacterial infection of the gums. In severe cases, gum surgery may be required.</p>
